How to Request and Obtain Your Medical Records
Before your marijuana evaluation appointment, it’s imperative that you obtain a copy of your medical records, indicating your diagnosis. In order to get medical marijuana in any state, you must first obtain a copy of your medical records.
If you arrive to your appointment without these records, your marijuana doctor will likely be able to request the records on your behalf, but this will delay the process of obtaining the medical marijuana recommendation needed to use cannabis legally in your state. It’s quick and easy to obtain your medical records, and it will help you get medical marijuana sooner.
Step 1: Contact Your Primary Care Doctor
Most practices require you to fill out a specific form to formally request your medical records. To obtain this form, simply call your primary care doctor’s office, or the office of the doctor who gave you the diagnosis, and request a copy. They should be able to deliver it to you by fax, email, postal mail or you may pick it up from the doctor’s office.
If the doctor’s office doesn’t have a specific form, simply write a letter to request your records. Be sure to include:
- Your name, including your maiden name (if applicable)
- Social security number
- Date of birth
- Address and phone number
- Email address
- Record(s) being requested
- Dates you were under the doctor’s care
- Signature
- Delivery option (pick up, fax, email, etc.)
What if I Don’t Have a Primary Care Doctor?
If you do not have a primary care doctor, are uninsured, have not had a check-up in a while, or cannot afford to see a physician, your medical marijuana doctor may be able to assist in finding an affordable clinic in your area that can give you an exam and diagnosis. Or, in some cases, your marijuana doctor may be able to take you on as a new patient and provide a diagnosis for you. In California and Colorado, patients may be issued a temporary recommendation pending the acquisition of medical records.
Step 2: Bring Medical Records to Your Marijuana Evaluation Appointment
Remember to bring your medical records form to your medical marijuana evaluation appointment. These documents are imperative to continuing your care and receiving a medical marijuana recommendation in every state.
